Kingston Health Sciences Centre is inviting applications from qualified, registered Physiotherapists, Chiropractors, and Nurse Practitioners based in the South East Local Health Integration Network (SE LHIN) to become contracted APPs.  From their respective practice locations, successful applicants will provide rapid patient assessment, education and self-management advice to patients with low back related pain.  APPs will be independent contractors compensated through a fee for service model.
Low back pain Rapid Access Clinics (RAC) are being implemented across the province to help people with low back related pain access the right treatment faster.  This work stems from the successful pilot program: Inter-professional Spine Assessment and Education Clinics (ISAEC) for low back pain (LBP). 
RAC LBP is an innovative, shared-care model in which patients receive rapid low back pain assessment, education and evidence-based self-management plans. Funded by the South East LHIN and hosted at the Kingston Health Sciences Centre, Hotel Dieu Site, it is designed to decrease the prevalence of unmanageable, chronic low back pain, reduce opioid initiation, and reduce unnecessary diagnostic imaging as well as unnecessary specialist referral.  The low back pain RAC is specifically for patients with: (1) persistent low back pain and/or related symptoms (e.g. radiculopathy, neurogenic claudication), 6 weeks to 12 months post-onset; and (2) recurrent / episodic unmanageable low back pain and/or related symptoms of less than 12 months post-recurrence.
As a member of the RAC LBP, the APP will:
  • act as the first and primary point of contact for consultation and assessment of referred patients
  • perform a thorough history and physical examination
  • provide a program of care to the patient with an emphasis on education, self-management and health promotion strategies
  • develop a collaborative care plan with the patient and their primary care provider and document progress against the care plan
  • identify signs and symptoms requiring diagnostic imaging/testing, referral to a physician specialist, or a change in the plan of care.
  • The APP will be connected to the Regional Advanced Practice Leader and Physician Specialists who will provide clinical guidance and advanced low back pain education. Additionally, all APPs will be connected through a centralized referral and case management system.
  • Please note: Successful candidates must complete both un-paid theoretical training (1 week online) and paid practical training (in person 2 days/week for 4 weeks) in low back pain topics.  Following the formal training period, participation in un-paid clinical education sessions is an expectation of the APP role.
Qualifications and Job Requirements:
  • Minimum 4 years of clinical experience following completion of an accredited, entry-to-practice program in a regulated health profession is required.
  • Including, at least 1 year of clinical experience in the musculoskeletal-spine area required.
  • Member in good standing of respective professional/legislative college at the provincial level required.
  • Minimum $5,000,000 Liability Insurance required
  • Member of related national/provincial professional association preferred.
  • Possession of a laptop/Chromebook, tablet, and printer.
  • Convenient practice location with access to the internet and preferably accessible via public transportation

Key Responsibilities:

Expert Practice
  • Act as the first and primary point of contact for consultation and assessment of referred patients with low back pain.
  • Review medical history/health status and conduct a comprehensive physical assessment.
  • Communication of a diagnosis.
  • Provide specialized care, education and counseling for patients.
  • Determine the suitability and eligibility of patients for surgical assessment.
  • Differentiate between inflammatory and non-inflammatory conditions.
  • Hold a broad understanding of indications for different evidence-based treatments for low back pain.
  • Identify signs and symptoms of undiagnosed medical conditions requiring a referral to diagnostic imaging/testing and/or referral to a Physician Specialist in accordance with established RAC LBP guidelines.
  • Initiate communication with primary care providers and Practice Lead to facilitate appropriate surgical and non-surgical referrals based on established RAC LBP assessment and referral guidelines.
  • Establish, communicate and document clinical impressions/medical diagnoses within defined scope of practice.
  • Develop plan of care in collaboration with the patient/family and primary care provider.
  • Document plan of care according to established RAC LBP guidelines.
  • Communicate internally and externally to facilitate plan of care (i.e. other health care professionals).
  • Evaluate and revise the plan of care in collaboration with patient/family and primary care provider.
  • Communicate specific medical diagnoses to primary care provider.
  • Utilize a patient-centered care approach consistent with patient goals, needs and values.
Teaching and Learning
  • Participate in RAC sponsored regional and central education and training events.
  • Collaborate with the RAC team to identify learning needs and those of patients and primary care providers.
  • Provide/receive formal and informal education internally and externally to/from RAC team members and primary care providers.
  • Facilitate knowledge transfer and evidence-based practice.
Organization Responsibilities
  • Act as an active member of the RAC.
  • Accountable to align activities and performance with strategic goals and objectives of the RAC.
  • Adhere to the RAC standards, policies and procedures, including centralized referral and clinic management system processes.
  • Participate in quality and performance management activities.
  • Participate in policy and procedure development.
Program Evaluation
  • In collaboration with RAC colleagues you will enable, implement, and conduct evaluations of clinical services related to the program.
